- Everything on this list is at least partially available in English. That's one of the pre-requisites for being on the list.
- I have not yet had a chance to do a proper exhaustive search of the media I've experienced over the years. While this list is fairly complete (outside the Fanfiction section), some things may have slipped through the cracks.
I'm always looking for new stuff. ContactMe if you know of something that should be on this list but isn't. I may have just not had time to check it out yet, but I may also have never heard of it.

I'm also interested in character-driven fiction involving age-regression, rapid aging and body swap which grants a child an adult's body, and cross-species transformation so I may make lists for those at some point in the future. The important part is for the story to provide a lot of interesting and creative character reactions to a known stimulus.

Footnotes
A Bug's Life: One of the jokes takes the form of Francis, a male ladybug who isn't too pleased about how often he is mistaken for a girl.
A.I. Love You: One of the A.I.s is number forty who has a permanently female body but switches between male and female personalities, complete with appropriate clothing. As an additional note, the personalities are flawed by 1990s Japanese standards. The female persona is as crude and dirty-minded as a boy and the male persona is as cleaning-obsessed and bossy as was supposedly proper for girls.
Akane-chan Overdrive: The main character's soul is a little loose... so he ends up getting knocked out of his normal body and occupying the body of a girl whose soul has run off.
All You Zombies: The main character is their own mother and father via time travel facilitated by their future self. The transformation is surgical and they are born intersexual, though the combination in question is, AFAIK, impossible.
Angel Sanctuary: Main character is the male reincarnation of a female rogue angel. At one point in the story, he has to wear the old body which has been preserved in crystal until then.
Boku no Futatsu no Tsubasa: A situational comedy involving a hermaphrodite who has never had sex-ed and got his/her first period years after entering puberty.
Boku no Shotaiken: After the main character attempts to commit suicide by leaping from a cliff, a mentally unhinged surgeon transplants his brain into the body of the sugeon's braindead wife.
Chameleon Jail: The main character's ability to transform isn't limited to himself. It is revealed that he has permanently transformed at least two men into women to drive the point home that they could never beat him, even if they broke of out prison and trained their butts off.
CUTE x GUY: So far, there are three major reversible transformers (two male, one female. all with different triggers) and one minor one-story transformer who had her memory wiped afterwards.
Detective Conan: One of the cases involves a guy who is pretending to a girl but I can't remember whether he was just cross-dressing or whether he had surgical help.
Disney's Mulan: Mulan is an old Chinese folk tale, but i've listed "Disney's Mulan" because that's the only version I've seen. As for the minor status of the MtoF cross-dressing, it may be a major plot point, but it occurs for very little time and is only a disguise of necessity, so in the case I decided to call it minor.
Disney's The Wind in the Willows: As with Mulan, the disney cartoon is the only version I've personally seen. The minor cross-dressing takes place when Mr. Toad dresses as a woman to escape prison.
DNA: The third case of Transformation is only partial. (the top half of the person's body)
El Goonish Shive: Transformation is done via a programmable transformation gun. Characters can and have used the associated scanning device to simulate body-swapping.
Full Moon Wo Sagashite: In episode 16, Takuto takes a turn wearing the illusion that Meroko normally wears to make the grandmother and housekeeper think nothing is amiss.
Gabriel: Due to an incompetent doctor, the main character is a girl with a fused labia who was raised as a boy with a different birth defect.
Get Medieval: The story follows a bunch of aliens who crash-landed in medival France. The cross-dressing minor character is a literate whore who decides to cross-dress and get a better job after swapping life stories with one of the main characters.
Ghost in the Shell: The main character is a cyborg. At the end of the movie, she winds up hiding in an effeminate but male body.
Gold Digger: A minor male character named Hanzo drinks a super-strength/vitality/etc. potion without realizing that it's only meant for women. He ends up permanently female and takes the name Hanza after calming down and accepting that there's no way to reverse it. She shows up several times throughout the later issues.
.hack//SIGN: Though he doesn't remember it until shortly before the end of the story allows him to log off, Tsukasa is actually a girl.
Harry Potter and the Half-Blood Prince: In order to act as lookouts without garnering attention, Crabbe and Goyle get polyjuiced into first-year girls. In addition, the title of the book refers to the fact that Snape is a half-blood and his mother's family name was Prince. This is noteworthy because Hermione makes a big production of of the loophole that Prince does not automatically equal male.
Here is Greenwood: Male members of the Kisaragi family are effeminate. Shun Kisaragi participates in a joke that involves him pretending to be a girl without actually cross-dressing. His little brother gets mistaken for a girl at first.
Honor among Thieves: Draco is only a boy because of a potion he's been taking since infancy to counteract the gender-tendencies of a bloodline with too much Veela ancestry... and with Lucius' death, he stops taking the potion for too long.
Hush: The FtoM cross-dressing is accidental. I should probably tag it as "Natural" since it's simply a case of the girl being mistaken for a boyfriend while in the presence of the cross-dressing guy.
If I Pay Thee Not In Gold: Due to the circumstances, it's extremely difficult to apply any modifiers to the type declaration. In the world of this story, succubi and incubi are one and the same. Sexual intercourse with the opposite sex triggers a physical sex change and therefore, *cubi are bigamous out of necessity. Since cubi have no "birth sex", the closest I can say is "BiDi", but that implies multiple characters. Also, as one of the main characters is an incubus, this should be (major, reversible), but as the events are portrayed, it's more of a (minor).
I.S. InterSexuality: This is a drama about a real-life condition. Roughly 1 in 2000 people are born not fully male or female. Examples include:
- Being physically female but genetically male (Androgen Insensitivity Syndrome)
- Being physically female/male except for having one testicle/ovary where an ovary/testicle should be (Hermaphrodites in the real sense)
- Various other variations.
Kedamono Damono: The main male character turns into a "pervy" (main female character's words) girl at night... and it's hereditary, so a similar change occurs to his brother and sister.
Kill me, Kiss Me: Two cousins (identical except for gender) trade places for a week when the girl discovers that the guy she has a crush on goes to her cousin's school. (She bribes her cousin into doing it)
Kon Puri: While the transformations are treated as if they are illusions by the character using them, their reality is proven when she turns into a bird and flies to school.
Love Letter From China: The song is sung by Ranma Saotome (See Ranma ½ in the anime or manga section) and throughout the song, people keep triggering his curse.
Mother's Day: This is a "Princess Ozma of Oz"-style story. The main character was born a girl and raised a guy. It's reversible because he/she is the princess of an alternate dimension and physical form is defined by which dimension he/she is currently in.
Naruto: Naruto's "Sexy no Jutsu" illusion technique does qualify for inclusion in this list. Also, there is speculation that Naruto can actually take it one step further and physically transform. This is primarily inspired by incidents like the time when Sasuke successfully threw him at Zabuza while he appeared to be a giant shuriken.
Neon Genesis Evangelion: I'm referring to the battle where Shinji had to wear one of Asuka's plugsuits.
Nerdcore: A character is revealed as being a disembodied spirit when he finally reclaims his body. However, the body thief happened to be a succubus who transformed it to fit her own form and, to quote the body's original owner, "It's going to take me forever to get it back the way it was, but that's okay. It's just going to be nice to be able to feel stuff again!"
No Side: While it is possible for a character to temporarily leave their new body (eg. for coercion-by-haunting), they are still limited to that body posession-wise.
Oh! My Goddess: Keiichi gains breasts from testing out potions ingredients on himself while trying to find something to cure an illness Belldandy has contracted. Then, a nature spirit named Troubador arrives in search of Urd (they used to be dating) and Keiichi has to cross-dress to hide from his potential jealous wrath.
Overlord: A Naruto story where Gaara is really a girl who has worn a male-form sand disguise since birth because Shukaku can't stand the thought of his host's true gender becoming known.
Ozy and Millie: There are several cases of "trading places", the most recent of which is done when they decide to cross-dress because it's the best they can do to see what it's like as the opposite gender.
Pitch Black: The cross-dressing in question is not exactly a major plot point, but it's less so a minor plot point.
Pokémon: James of Team Rocket has worn a good many disguises that count as cross-dressing... including a specially-designed bikini with inflatable breasts.
Pretty Face: The guy who cross-dresses does so as a result of a chain of events set off by a crazy plastic surgeon giving him the face of the girl he likes.
Shonen Linda: Apparently only three pages. (It's in the form of a full page and four newspaper strips)
Sparkling Generation Valkyrie Yuuki: The male-to-female transformation is a non-reversible side-effect of the main character gaining a reversible "magical girl" transformation.
Tales of the FoxCat: Naruto taps the Kyuubi's power and (quite painfully) transforms himself physically into a girl in order to get into Ranma's (It's a very well-written crossover) Kunoichi training. (Because it's got more interesting stuff than the from-the-basics martial arts training Ranma is already giving him)
Tenshi na Konamaiki: Megumi was actually born as a girl. She and Miki just had their memories altered by the little demon... that still counts as transformation from their perspectives.
The 'Boy' Who Lived: This story explores how canon might differ if Lily Potter gave birth to a daughter named Hallie Potter but, for the sake of protecting the child, the general public was told that the child was a boy. The illusion in question is cast by Dumbledore "to give Hallie a chance to win peoples' loyalty and friendship before they find out". (In essense, an attempt to side-step prejudice)
The Darkstarr Chronicles: At the beginning of the story, the main character (born male) has been living a new life as a female bounty hunter for roughly two decades. Shortly after the story starts, s/he visits a genetic resequencing parlor to regain male form.
The Dosadi Experiment: A male and female character end up telepathically linked in such a way that they can trade bodies. Later, when the female character is killed, the male character manages to save her, resulting in their cohabitation of his body.
The Virus: This is the Ranma pandemic story. The one where Mihoshi's talent for messing things up and Ranma's penchant for chaos team up to create "Nerimavirus" which then infects the whole world... and it's well written!
Thousandstar: Thanks to the similarity between gender-swapped clone-twins and some cross-dressing, a female mind ends up 'piggybacked' in a male body... something that shouldn't be possible for travel by mind transmission.
Tokyo Crazy Paradise: The story takes place in a future Tokyo where the only thing more common than murder is crimes against women. The main character is the daughter of two police officers who was raised as a boy for her own protection. At one point, it's revealed that she feels as if she's cross-dressing while wearing a dress as part of a disguise. (it's also revealed that she hates female clothing because it's "un-practical and drafty")
Triquetra Cats: The transformation is a major plot point, but it fails the "major portion of the story, time-wise" test. Basically, the main characters are three girls who grew up as boys for their own protection. Once they're transformed back, that is all but forgotten.
Urusei Yatsura: At two different points in the story, Ataru is hit by transformation rays as a way to show that gender has no effect on his perversion. The first makes him mentally female in a male body. The second makes him physically female with a male mind.
Venus Ascending: An exploration (with permission) of what Venus Envy might be like in a Star Trek-like setting.
Wandless Magic: A Harry Potter view of the universe created in Space Pirate Hamster's "The Virus". (A fun mega-crossover fanfic where the greatest chaos-makers of both Tenchi Muyo and Ranma ½ end up accidentally making Ranma Saotome's curse into a pandemic)
WickedPowered: Main character is turned into a girl as a disguise. One of the jokes reveals that the reason he's so screwed up is that he accidentally mimicked the plot of "All You Zombies" and became his own mother and father.
You Did What!: The third in an excellent humor series by nonjon. The gender-bending is the result of pranking of various sorts. Make sure you read the first two stories in the series ("Where in the World is Harry Potter?" and "The Untitled Cheekquel Project") before you read this. Otherwise, it won't make any sense.
